Interfaith CarePartners® is a non-profit organization that works with partner congregations to assist their development of service programs that manifest each congregation's commitment to member care that serves both its own families and as an outreach program in the wider community.

Interfaith CarePartners'® principals created the Care Team® model of in-home support for families with special needs in 1985. Congregation-based Care Team® projects are linked together by Interfaith CarePartners in an interfaith network of partner congregations as a basis for team training, continuing education, and volunteer service.

A Care Team® is a group of 12 or more laypeople from a congregation who offer support to individuals and families affected by chronic or terminal illnesses or disabilities. Care Team® members work in groups, sharing service activities so that no team member is solely responsible for the level of care that may be required.

- Emotional, physical, practical, and non-proselytizing spiritual support
- Limited transportation - to worship, shop, or clinic
- Friendly visits and companionship
- And, for caregivers of ill or disabled loved ones, respite opportunities that provide a break without worry.
Interfaith CarePartners supports congregational Care Team® projects for:
- ALZHEIMER'S & other dementia-related disorders
- HIV / AIDS
- SECOND FAMILY (frail adult, other than dementia-related)
- KIDS' PALS

This exemplary program achieved the milestone of 1 million hours of service in June of 2002. The remarkable story of over 9,400 Care Team® members who, since 1985, have tirelessly donated over 1.30 million hours of in-home care and support of care partners is an unprecedented level of compassionate caregiving.
